Neuropathic pain can be hard to diagnose but is important to identify and treat in dogs and cats. In humans, only half of the patients with neuropathic pain find partial relief. The numbers are likely much worse in animals. In neuropathic pain, the peripheral nerves drive all changes in the “pain sensing system,” leading to neurogenic inflammation. This type of pain often causes behavior changes due to abnormal sensations (eg, allodynia), which should increase suspicion of pain and warrant treatment. If neuropathic pain is suspected, trial therapy is recommended. Several classes of drugs, including tricyclic antidepressants, gabapentin, amantadine, and topicals such as lidocaine, may be effective, as can acupuncture. Studies support the potential of CBD/CBDA products for treating neuropathic pain and osteoarthritis. Hemp-derived cannabinoids are legal at the federal level, but state regulations vary. Protect yourself and your patients by using products that have pharmacokinetic data, safety and efficacy studies from more than one laboratory, and a full Certificate of Analysis. Not all hemp compounds are the same, and products may contain a wide variety of compounds. A full-spectrum hemp product that fits the previously mentioned criteria is more likely to be effective. Acidic cannabinoids (eg, CBDA) show superior absorption and retention over decarboxylated forms (eg, CBD).
