Pneumoperitoneum can be an important clue to the presence of GI perforation, but finding small amounts of free gas on abdominal radiographs can be difficult. In this study, radiographs produced using different views and techniques were evaluated for the presence of air by board-certified radiologists and nonspecialist clinicians.

Accuracy in detecting free air was higher using standard left lateral radiographs (80.4%) and in lateral radiographs produced using a horizontal beam (78.9%), as compared with standard ventrodorsal (60.7%) and ventrodorsal horizontal (65.5%) beam projections. In addition, specialists were able to find free air more accurately than nonspecialist clinicians. {br}This study suggests that a practical approach to detecting pneumoperitoneum includes a standard left lateral radiograph and review by a board-certified radiologist.
