In this study, 22 dogs with confirmed hyperadrenocorticism who had been treated with trilostane for at least one month were assessed for dose adequacy using a variety of methods, including ACTH stimulation testing, evaluation of urine specific gravity, urine cortisol:creatinine ratio, and baseline serum cortisol concentration. The result of each test was evaluated and compared with owner assessment of clinical signs at home.

Dogs were classified as underdosed, adequately dosed or overdosed based on owners’ perception of the patients’ clinical signs. Laboratory testing showed no significant differences between adequately dosed and underdosed dogs. ACTH stimulation testing did not correlate with clinical signs and did not help identify dogs that required a dose increase. Based on these results, dose adjustments should be made based on the needs of the individual patient and observation of factors such as thirst, urination, appetite, behavior, and clinical signs of illness in addition to laboratory testing.
